Carcinoma ex pleomorphic adenoma of the submandibular gland with renal metastases composed exclusively of metastasizing pleomorphic adenoma: a case report

Background: Carcinoma ex pleomorphic adenoma is an exceedingly rare neoplasm of the submandibular gland with an overall poor prognosis. Metastatic deposits of carcinoma ex pleomorphic adenoma are either composed exclusively of malignant elements or contain mixed benign and malignant components. Case Presentation: We describe the case of a 63-year-old male with a carcinoma ex pleomorphic adenoma of the submandibular gland with metastases to the kidney. The patient had a history of having undergone two previous surgical resections for a left submandibular tumor. Right partial nephrectomy was performed and histological examination of the resected specimen revealed benign pleomorphic adenoma with no histological evidence of malignancy. This pattern of spread has been described before with metastasizing pleomorphic adenoma. We subsequently performed left submandibular gland excision and made the final pathological diagnosis of carcinoma ex pleomorphic adenoma. Conclusions: This unusual case suggests that carcinoma ex pleomorphic adenoma and metastasizing pleomorphic adenoma could represent different stages along a common biologic pathway of malignant transformation of pleomorphic adenoma.
